Areas of work for Continuity of care
The KPI Programme provides information for kaimahi working in two key settings: inpatient and community services. Our five focus areas of work cover: adult, child and youth, non-government organisations (NGOs), adult forensic, and youth forensic services.
Continuity of care dashboards
The continuity of care indicator helps us see how well Te Whatu Ora district services and NGO mental health providers work together to support tāngata whai ora before, during, and after an acute inpatient stay.
This indicator was developed in response to feedback from tāngata whai ora, who said they often experience disconnected or inconsistent care. It is designed to support reflection, learning, and improvement across the mental health sector. Note: you must be registered and sign in to use the KPI Programme data dashboards.
The settings for this indicator are inpatient and community.

KPI Programme kaitaki
The KPI Programme is supported by a group of people, or kaitiaki, working in the five focus areas. They work across the motu, and use their speciality skills and knowledge to champion the programme and drive continuous service improvement within their services and network groups.

Why continuity of care matters
When tāngata whai ora move between services, for example, from an NGO setting into an inpatient unit and back into the community, it’s important their care stays connected and consistent. Good continuity of care means people are supported by the right services at the right time, without gaps or confusion.
When district services and NGOs work well together, it can lead to fewer readmissions, shorter hospital stays, better recovery and wellbeing outcomes, stronger whānau and community support
This indicator helps identify where those connections are strong and where services can improve how they work together.
Age groups
All indicator dashboards provide filters that allow you to choose the age groups relevant to your area of work. Note the default setting for dashboards is ‘All’ age groups.
Data source
All indicator dashboards are created using information from PRIMHD, the single national data collection for mental health and addiction services in Aotearoa New Zealand.
Information available through Month YYYY, sourced from the DD MM YYYY refresh of the PRIMHD DataMart.
Note: Northland has low or no data since April 2025. Tairāwhiti have lower inpatient volumes from March 2025. West Coast have lower inpatient volumes from July 2025.
